Our new site at Crouch Vale Medical Centre has full disabled access including a lower reception desk, lift and wide doorways.

Dear patients,

We would wish to inform you that from Monday the 22nd of  June 2020 all appointments can only be booked by telephone. We are continuing to follow a ‘telephone triage first’ policy and clinicians will make the decision on who they feel should be brought in for a face to face consultation after the triage, if needed. We ARE NOT  open for patients to walk in and book appointments and will not accept patients queuing outside to book an appointment. If you so choose to walk in to book appointments you will be asked to return and ring us back for a telephone triage.

We have had to modify our ways of working to ensure patient and staff safety. We  are now looking to normalise our patient care. We appreciate this ‘new normal’ is a novel way of working with us and sincerely appreciate the forbearance and patience this has needed and which you have provided thus far. The pandemic has engineered newer ways of working in the form of telephonic,video consultations etc and we intend to continue to rely on them on as an effective and safe means of communication with you.

If you are asked to attend for any appointment with the Nurse or GP please make sure you wear a face covering to the appointment. Do come alone unless you are bringing a baby/child or you need help with your mobility or you need a carer or friend to attend the appointment with you.

Please note that any images you send in for review need your consent for them to be  saved onto your clinical records. This will be securely stored and will help aid in your diagnosis and management. The images will not be used for any other purpose. If you choose not to have these images saved onto your clinical records please let us know and they will be deleted.

If you have a cough, raised temperature or loss of taste and/or smell, PLEASE DO NOT ATTEND the surgery, go online to NHS111 or call NHS 111 for advice and further treatment.
